| AFFILIATE MEMBERSHIP | A manufacturer, distributor, or supplier of products or services purchased by dental laboratories. | $150 Annually |
| ASSOCIATE MEMBERSHIP | Individual employed by member firms and privately employed technicians and educators of CDLA approved schools. | $150 Annually |
| DOCENT MEMBERSHIP | Individual wishing to support the dental laboratory by limited membership. | $45 Annually |
